What is a Hairline Hair Transplant?

A hairline hair transplant takes DHT resistant hair follicles from the back and sides of the head (the donor area) and implants them into the frontal hairline and temple zones. The goal is a restored hairline that is indistinguishable from one that was never lost. Not just full, but naturally shaped, with the correct angle and direction for each individual follicle.

The most important technical detail in hairline work is one that patients rarely see discussed: natural hairlines are never perfectly straight. A hairline that follows a ruler straight edge looks immediately artificial under any lighting. The defining characteristic of a natural looking hairline transplant is deliberate micro irregularity, a softly irregular front edge created using single hair grafts, placed at an acute forward angle, that mimics the way hair actually grows at the temple and frontal zones. The difference between a result that is detectable and one that is not almost always comes down to this detail rather than the number of grafts used.

Three surgical methods can be used to restore a hairline: FUE hair transplant, FUT hair transplant, and DHI hair transplant. More information on each method is provided below.

Who is Suitable for a Hairline Hair Transplant in Leeds?

Hairline transplants are available to men, women, and transgender patients at our Leeds clinic.

Men

For male patients, a hairline hair transplant is most appropriate at Norwood stage 2 or 3, where recession has begun at the temples or the central hairline but has not yet progressed to significant crown thinning. Treating the hairline at this stage preserves donor grafts for any future work and produces the most natural-looking result, because the existing hair density behind the transplanted zone is still high enough to blend seamlessly.

If crown involvement is already present, a two stage plan is usually more sensible: hairline first, with the crown addressed in a separate session once full growth from the first procedure has been assessed. Committing all available grafts to both areas in a single session frequently under-treats both zones.

Women

Female patients typically come to us with one of two presentations: a naturally high hairline from birth (not caused by any hair loss condition) or early stage diffuse frontal thinning. In the case of a congenitally high hairline, a transplant can lower the frontal line by 1-3 cm, shortening the perceived forehead and restoring natural facial proportions. Because no progressive hair loss condition is present in these cases, the result is stable long-term without the need for ongoing medication.

Transgender Patients

For MTF transgender patients, a hairline transplant addresses both recession from any prior male pattern loss and the reshaping of a typically masculine hairline into a softer, more rounded feminine profile. What Surgical Methods Do We Offer?

At the Leeds Hair Transplant Clinic, we offer three surgical methods for hairline restoration. Each has specific advantages depending on the patient’s hair type, the extent of recession, and the level of precision required.

Follicular Unit Extraction (FUE)

FUE is the most popular hair transplant method used at UK hair transplant clinics and the one we perform most frequently at our Leeds clinic. Individual hair follicles are extracted from the donor area one by one and implanted into the hairline. FUE leaves no linear scar, has a recovery period of 10-14 days, and is suitable for all stages of hairline recession. FUE hair transplants in Leeds allow patients to wear their hair short without any visible scarring at the donor site.

Direct Hair Implantation (DHI)

DHI uses a CHOI implanter pen to make an incision and implant each follicle individually at the same time. This reduces the time each graft spends outside the body, improving survival rates, and gives the surgeon more precise control over the angle and direction of each implanted hair. For hairline work, where angle and direction are critical to naturalness, DHI is often the preferred method for the front edge of the hairline.

Follicular Unit Transplantation (FUT)

FUT involves removing a strip of scalp from the donor area, dissecting individual follicular units from it under a microscope, and implanting them. It allows a large number of grafts to be harvested in a single session and can be appropriate for patients with very limited donor density. However, it leaves a linear scar at the donor site, making it unsuitable for patients who prefer to keep their hair very short. It is less commonly recommended for hairline only cases and is discussed during the consultation where relevant.

The Day of Your Hairline Hair Transplant in Leeds

Initial Consultation and Anaesthetic

On the day of your procedure, you will begin with a pre surgery consultation with your surgeon and team. The final hairline design is confirmed at this point as you will see exactly where the new hairline will sit before anything begins, and any adjustments are made now. Local anaesthetic is then administered to numb the donor area. You will remain fully awake throughout.

Extraction Phase

Hair follicles are extracted from the donor area at the back and sides of the head. Most hairline procedures use between 1,500 and 2,500 grafts, depending on the extent of recession and the desired density. Grafts are categorised as singles, doubles and triples. Single-hair grafts are reserved for the very front edge of the hairline to create that soft, natural transition whilst doubles and triples provide density behind the front row. The extraction phase typically takes 2-3 hours.

Incision Phase

The surgeon first numbs the recipient area with local anaesthetic. He will then make a series of small incisions into the recipient hairline area using a precision steel or sapphire blade. On hairline work, incision angles are typically between 10 and 30 degrees, far more acute than in crown surgery, and follow the natural direction of frontal hair growth. The design of the incision pattern determines the final result. This phase is performed exclusively by the surgeon.

Implantation Phase

The prepared grafts are implanted into the incision sites with precision. The positioning of single hair units at the very front, grading into denser multi hair units behind, creates the visual effect of a fully natural hairline. Once complete, your surgeon walks you through your aftercare protocol for the following 10–14 days and provides a full aftercare kit.
