What is a Transgender Hair Transplant?

A transgender hair transplant uses the same FUE or DHI surgical technique as any other hair restoration procedure. The difference lies entirely in the goal: the hairline is designed, shaped, and restored to reflect the patient’s gender identity rather than simply to reverse hair loss.

For MTF patients, the surgery typically addresses male pattern recession in the temples and frontal hairline that occurred before or during transition , creating a softer, rounder, lower frontal line that reads as feminine.

For FTM patients, the goal is often the opposite: reshaping a naturally lower or more curved female hairline into a higher, more defined masculine profile. FTM patients who use testosterone as part of hormone replacement therapy may also experience androgenetic hair loss over time, and a transplant can address this directly.

In both cases, the donor follicles come from the back and sides of the scalp. Because these follicles are genetically resistant to DHT, they are permanent regardless of ongoing hormone therapy.

MTF and FTM Hairlines , the Design Difference

The most clinically important detail in transgender hair transplantation is hairline geometry. The anatomical differences between a typical male and female hairline are specific and measurable , and getting them right is what produces a result that genuinely affirms gender rather than one that simply fills in hair.

A female hairline typically sits 5–6 cm above the brow, follows a soft, rounded arc with minimal temporal recession, and has no defined peaks at the temples. The transition from hairline to temple is gentle and continuous.

A male hairline is typically set higher (6–8 cm above the brow), forms a characteristic M-shape with defined temporal recession points, and has a sharper, more angular transition at the temples.

For MTF surgery, the procedure moves the effective hairline downward and eliminates the temporal recession points , rounding the overall shape. For FTM surgery, the hairline may be raised slightly and the temporal angles defined. In both cases, the incision pattern and graft placement follows the specific growth direction of the target gender’s hairline, not the patient’s existing one. This geometric planning is the difference between a natural-looking result and one that is simply fuller , and it requires both surgical precision and a clear brief from the patient before a single incision is made.

Who is Suitable for a Transgender Hair Transplant in Leeds?

Suitability for a transgender hair transplant is assessed at your free consultation. The two key factors are the same as for any hair transplant: sufficient donor density at the back and sides of the scalp, and a clearly defined recipient zone to work within.

Additional considerations specific to transgender patients include:

  • MTF patients , Patients who have experienced male pattern recession before or during transition are typically good candidates. If you are on oestrogen and testosterone blockers, any ongoing DHT-driven loss should be largely stabilised before surgery is planned. We will discuss your hormone therapy history at consultation.
  • FTM patients , Patients using testosterone as part of HRT may be experiencing androgenetic thinning. Surgery can be performed once the pattern of loss is stable and predictable, so it can be planned around without risking the result.
  • Patients not on HRT , A transplant is still possible. The surgical plan will account for the likelihood of future hair loss and preserve donor grafts accordingly.

There is no minimum or maximum age requirement. We treat each patient individually and do not make blanket eligibility decisions before a proper assessment.

Surgical Methods for Transgender Hair Transplants

Follicular Unit Extraction (FUE)

FUE is the most commonly used method at our Leeds clinic for transgender patients. Individual follicles are extracted from the donor area and implanted one by one. FUE leaves no linear scar and allows patients to keep their hair short at the donor site , an important consideration for many trans patients. FUE hair transplants in Leeds are performed exclusively by our GMC-registered surgeons.

Direct Hair Implantation (DHI)

DHI uses a CHOI implanter pen to extract and implant each follicle in near-simultaneous steps. This gives the surgeon greater precision over the angle and direction of each graft , which is particularly valuable in transgender hairline work, where the exact geometry of the result matters more than in a standard restoration case.

Follicular Unit Transplantation (FUT)

FUT is an option in cases where maximum graft yield is required in a single session. It leaves a linear scar at the donor site, so it is generally recommended only for patients who wear their hair long enough to conceal it. It is discussed at consultation where it is clinically appropriate.

Transgender Hair Transplant Cost in Leeds

Transgender hair transplant procedures at our Leeds clinic start from £3,000 for smaller cases and typically range between £4,500 and £6,500 depending on the extent of work required. We do not price on a per-graft basis. The cost of a hair transplant in Leeds is determined by the surface area being treated and the complexity of the hairline design. Finance plans are available, allowing you to spread the cost over 12 months at 0% APR or over a longer period if preferred.

Hair Loss Medication for Transgender Patients

For MTF patients, finasteride is sometimes used as part of aftercare to prevent further DHT-driven thinning of native hair surrounding the transplant. Finasteride is not appropriate for FTM patients. If you are on oestrogen and testosterone blockers, this should be discussed with your prescribing clinician as well as our surgical team before any medication is added. We will always work in coordination with your existing healthcare providers.

For FTM patients on testosterone, minoxidil may be discussed at consultation as a way to support density in areas not covered by the transplant.

It was an outstanding experience from start to finish. Naturally, I was unsure on which organisation to choose for the procedure, but HSHTC came across as professional and caring, as well as having a wide range of availability. There are negative reviews on all businesses, and this drives customers like myself away. However, something told me this would likely be someone with unrealistic expectations, or not reading into the procedure as a whole. I chose to ignore the negatives and read the positives. Sara the coordinator was absolutely amazing! She outlined everything and made it clear of what is expected and what is not. After a great consultation, I was passed to the next stage of meeting surgeon Dr Ditta. He presented extremely knowledgeable and passionate about what he does, so I decided to go ahead with the procedure. The surgery day was expected to be a long, but rewarding day. It started with having to shave my head (mandatory requirement for this procedure). I’d say to anyone the thought is worser than the event, and to just do it for the positives that will follow. Dr Ditta and his team were extremely caring and sensitive, especially with briefing before any painful situations pre anaesthetic. After that, I was allowed to top up when requested and leave them to do what they do best. The day as a whole was enjoyable and interesting to speak to the team to learn more about the procedure and get to know them. Lunch was provided (yay). It was lovely of them to offer, and you definitely need to eat mid procedure to keep energy levels. At the end of the day, I was given clear instruction of how to have the best success rate. It was through a video and in person. If you have questions, ask as it’s what the team is there for. The first few days of recovery can be quite awkward and uncomfortable, but I’d recommend booking 2 weeks off work to fully recover and let the grafts set. Once you get use to the sleeping situation, it’s easy and a lovely break from reality (put pillows below your knees and stack below head for pure comfort!). The aftercare products was extremely useful and easy to use. The grafts will fall out after a few weeks, it’s expected and you’re told this plenty of times!! Having a healthy lifestyle makes your hair grow quicker and thicker, so swap your diet to good healthy hair foods. When your transplanted hair grows back, it’s an amazing feeling, but only the beginning. As the months pass by, you’ll notice a massive improvement. During this process, you’ll have regular aftercare and the chance to raise any concerns. I personally didn’t have any issues and was just grateful to still be in contact and told everything is going as it should. I’m 6 months post surgery and I’m seeing the benefits hugely, and it’s not even reached maximum results yet, as this is one year. Thanks for an amazing service!

Having given some time and thought about having a Hair Transplant, I contacted Harley street hair transplant clinics. The consultant (Sara) explained everything regarding the procedure with Dr.Ditta and the technicians. From the beginning, Dr.Ditta explained the days events and how the procedures would be done beginning with taking a photo of my head and showing me where the grafts would be taken from and where they would be placed. The procedures would be done and completed in two parts, with a lunch break in between. Once completed, Dr.Ditta went through the aftercare, explaining what to do each day for 10 days. I was given capsules, shampoo, sponge, neck pillow for sleeping with and information regarding how to look after my hair. Dr. Ditta and his team were welcoming and friendly ensuring I was looked after throughout both parts of the procedure. Sara maintained regular contact to make sure recovery was going well. The results were great, patience is needed as the grafts start to grow, and then you can see the results. Extremely happy with everything, from start to finish.

Would highly recommend! Everything went very smoothly, communication was excellent throughout. I travelled from Edinburgh to Manchester for the procedure and all travel and accommodation costs were covered. Dr Ditta explained everything clearly and delivered clear and detailed post-op instructions. I was delighted with the whole experience and results so far are very promising!

Overall, my experience at the clinic was positive. Dr. Ditta was exceptional — he made me feel comfortable and at ease throughout the procedure, and his professionalism was truly reassuring. However, I was disappointed with one member of the team who, in my opinion, did not fully follow health and safety protocols. She had her hair down without a surgical cap and was using her phone while wearing gloves, then proceeded to touch my hair without changing them. This raised concerns about hygiene and cross-contamination. Her manner was also not very pleasant, which made the experience less comfortable. Although I was initially worried about the risk of infection, I felt more at ease by the end of my visit. I hope the management reviews this matter to ensure that strict hygiene and professional standards are consistently maintained. Aside from this, I was very satisfied with Dr. Ditta’s care and look forward to seeing my results.

Before the procedure I had online and face to face consultations with no obligations. These gave me all the information I needed and really put me at ease and gave me the confidence to proceed. Everyone was really knowledgeable and friendly and clearly very experienced. Booking the procedure was quick and easy and I got a date that suited me. The procedure itself went really smoothly I was able to put headphones in and listen to podcasts and was very comfortable. Afterwards I was given really detailed notes on what to do and what would happen and everything went exactly as planned. It was so reassuring knowing that advice was only a text message away and responses were always immediate. I'm two weeks post procedure now and everything has gone back to normal and the recovery is going just as planned. I'm so glad I did this and chose Harley Street Hair Transplant!

Great clinic with friendly and experienced team. I wanted my surgery done in the UK and found the clinic on Google. Process started with virtual consultations with patient advisor and then with the surgeon. FULL costs were given up front, with good finance options available. On the day I designed a new hairline with the surgeon and ended up getting about 1600 grafts iirc, which was good value for money. Clear pre- and post-op instructions given and recovery has gone as expected. Now waiting patiently for regrowth :) - Grant
