At Hair Transplant Leeds, DHI (Direct Hair Implantation) is offered as an alternative to standard FUE for cases where precision placement of each follicle matters most. This page explains how the technique works, where it excels, and how to decide between DHI and standard FUE.
DHI Hair Transplant in Leeds
DHI extracts hair follicles from the donor area using the same FUE punch technique, but implants them using a specialised device called a Choi implanter pen. The pen combines the incision and implantation into a single step: the surgeon loads a follicle into the pen’s hollow needle, places the tip against the scalp, and simultaneously makes the incision and deposits the graft. No separate channel creation is required.
The result is greater control over the angle, direction, and depth of each implanted follicle. This matters most in areas where natural hair growth follows very specific geometry, particularly the hairline, where the difference between a result that looks natural and one that does not comes down to the precise placement of individual follicles.
DHI is also used when treating areas with existing hair, because the pen allows the surgeon to place grafts between existing follicles with minimal risk of transection.

How the DHI Method Works
Extraction
Donor follicles are extracted individually using the same 0.75mm to 0.85mm punch device as standard FUE. The donor area is shaved and local anaesthetic administered. The surgeon performs the extraction; the technician team manage and sort the grafts.
Graft Loading
Each extracted follicle is loaded individually into a Choi implanter pen. The pen holds one graft at a time. Because grafts are loaded and implanted in sequence rather than prepared in bulk, they spend less total time outside the body compared to standard FUE. This can improve graft survival rates, particularly in large sessions.
Implantation
The surgeon places the pen against the recipient area at the planned angle and depth and depresses the plunger. The incision is made and the follicle deposited simultaneously. No prior channel creation is needed, so there is no window of time between the channel opening and graft placement during which channels can close or swell.
This process is repeated for every follicle. DHI sessions take longer per graft than standard FUE implantation because of the sequential loading and placement, but the precision advantage offsets this for appropriate cases.
DHI vs FUE: When to Choose DHI
The extraction phase is the same in both methods. The difference is entirely in implantation.
DHI is preferred when precise angle and direction control is critical. This is particularly true for hairline hair transplants, where the forward angle of each follicle at the very front edge of the hairline determines how natural the result appears. It is also preferred when adding density to areas with significant existing hair, where the pen allows accurate placement between existing follicles without damaging them.
Standard FUE is more efficient for large surface areas such as the crown, where the priority is graft count and density coverage rather than micro-precision at a defined front edge. For a significant crown hair transplant, standard FUE is often the faster and equally effective option.

DHI Hair Transplant Cost in Leeds

Because DHI takes longer per graft at the implantation stage, very large graft count sessions may carry a slightly different cost to an equivalent FUE session. DHI Recovery: What to Expect
The recipient area will be red and slightly swollen. Crusting forms around each implanted follicle. Sleep elevated, begin saline spraying as instructed, and avoid touching the recipient area. Because no pre-created channels are present, the grafts are well secured from day one.
Shock loss (shedding of the transplanted hairs) begins between weeks 2 and 6 in the majority of patients. This is normal. The follicles remain intact below the skin and will produce new growth. Resume gentle washing by day 10 to 14.
Initial growth appears around months 3 to 4. Coverage becomes more visible between months 6 and 9. DHI results in the hairline zone often become apparent earlier than crown results due to the shallower recipient depth.
Full DHI results are typically visible at 12 months. At your follow-up appointment your surgeon will review the density and direction of growth and assess whether any further work is beneficial.

Neither is universally better. DHI offers greater precision at the implantation stage and reduced time-outside-the-body for each graft. FUE is more efficient for large surface areas and equally effective for crown restoration. The best method depends on the specific area being treated and the degree of precision required. DHI uses the same FUE punch extraction method, so donor-site scarring is the same: small dot scars that are not visible with hair at a normal length. There is no linear scar. The recipient area heals with no visible scarring.
Yes. DHI is particularly well suited to facial hair restoration because the angle and direction of each implanted hair must closely match the natural growth pattern of the target area. For a beard transplant or eyebrow transplant, precise angle control is as important as graft count. The Choi pen gives the surgeon that level of control.
